Locum Anticoagulation SHO required for an NHS Trust in South West London.
This role is to start ASAP and will be 8 hours per day on Tuesday & Wednesday for 4-6 months.
Candidate would require:
* Knowledge of Direct Oral Anticoagulants (DOACs), indications and contra indications, initiation and dosing, counselling of patients, possible adverse effects, and the management of these.
* Knowledge of Vitamin K antagonists (VKA) including Warfarin and Acenocoumarol, indications and target INR’s, contraindications, initiation, risk factors and algorithms, dosing, and monitoring of the same. Ability to counsel patients on VKA’s, adverse effects and its management, plus knowledge of the reversal of VKA’s for patients with high INR’s. An understanding of Bridging therapy with LMWH or unfractionated heparin for sub-therapeutic INR’s, plus the ability to orchestrate and manage the interruption of Anticoagulation for surgery and invasive procedures.
* Knowledge of LMWH and unfractionated heparin, contraindications, cautions, and its monitoring. An understanding of prophylactic and therapeutic dosing including prescribing for oncology patients, and possible complications including Heparin induced thrombocytopenia (HIT)
* Ability to prescribe
* Good interpersonal skills.
* Able to work in a fast-moving pressure environment
* Able to work on own initiative
In order to apply you must meet the following minimum criteria:
- full GMC registration with a licence to practice
- have the Right to Work in the UK (visa sponsorship not available for this post)
- have at least 3-6 months prior UK experience in a clinical setting
